Student COVID Vaccination Upload

To submit a copy of your vaccination card using the KVApp:

Once you’ve received your first and then second dose of either the Pfizer or Moderna vaccine, or your single dose of the Johnson & Johnson vaccine, you’ll need to upload proof of your vaccination using the KVApp following the instructions below. 

Please do not email your proof of vaccination.  If you have concerns or issues using the KVApp to supply proof of vaccination, please contact Dean Kevin Casey at 207-453-5141 or kcasey@kvcc.me.edu.

KVCC’s student vaccination submission form is available on the Campus Guide in the KVApp. To navigate to the Campus Guide, tap the second icon on the bottom tray (see image below). The vaccination form submission will ask you to take a photo of your vaccination card or upload a photo from your image library. 

 After you submit your proof of vaccination, you will receive a yellow Health Pass showing you have submitted the first of two vaccination records, or a blue Health Pass showing you’ve uploaded your single-dose Johnson & Johnson card, or both doses of a two-dose series.  You will need to display your Health Pass at each building’s Welcome Station to be permitted entry.

Please note that students will have until the first day of the Fall semester to submit proof of at least their first dose, and 30 days from the date of their first in-person class to submit proof of full vaccination.  If there is an issue with your submitted proof, you will be contacted by the College through your official KV email address within 24 hours.

 If you have not yet downloaded the KVCC Mobile App, it is available for free for all Apple and Android Devices by going to http://kvcc.campusapp.com/. After downloading the app, choose Log In and enter your KVCC credentials (see image, below).
