Accommodation Procedure

·    You must identify your disability status in a timely manner with the

 Disability Counselor. 

·    Make an appointment with the Disability Counselor.  Do this early in the

 semester to ensure you start out on the right foot!

·    Submit current documentation from an appropriate professional.

·    Make a request for an accommodation.  Accommodations are

     determined by  the documentation provided, the critical elements of a

     program or course, and the student’s request. 

·    Complete an Accommodation Form.  A new form is required for each

     new  semester/course.

·    Provision of the accommodation is negotiated with the student and the

 instructor.  For example, extra time may be provided by the instructor

      or the Disability Counselor.

·    Maintain ongoing communications with faculty and staff regarding the

 effectiveness of the accommodation.  We won’t know, if you don't tell!
