Forms for Students

Student Handbook
This document outlines the definitions of Service-Learning, goals of Service-Learning, the process of developing a Service-Learning project, guidance on reflection.

SL Placement Agreement
This form is used to document the creation of a Service-Learning project and is signed by the Student, the Community Partner, and by the Faculty.

SL Summary Sheet
This form is used to document the Service-Learning experience and to track specific demographic and service indicators for the CCE.

SL Supervisor Evaluation
This form allows the Community Partner the opportunity to evaluate the student who has completed service.
