1. I certify that I do not owe a refund on any educational grant or loan, am not in default on any educational
loan, and have not borrowed in excess of federal Title IV loan limits at any institution.

2. I acknowledge that the total financial aid I receive cannot exceed my
financial need as calculated by the Financial Aid Office, and that any excess
funds must be repaid. If I receive funds for which I am ineligible, due to my
error or KVCC’s error, I must repay the monies in full immediately. I
understand that I cannot receive Financial Aid awards from KVCC and another
school for the same period of enrollment even if I am concurrently enrolled at
both schools. If double payment occurs, I am responsible for full repayment of
the excess award.

3. I understand that the final determination of financial aid eligibility will be
based on the number of credit hours I am taking at the end of the add/drop
period. If I reduce the number of credit hours after that date, I may be
responsible for repaying all or part of any awards I receive. I must tell the
Financial Aid Office of any academic changes that may affect my eligibility,
such as: course add/drops, change in number of credit hours attempted, grades
of incomplete, withdrawal from courses or from KVCC, transfer into another
program or to another college.

4. I must tell the
Financial Aid Office of
any changes in resources, (i.e. private scholarships, veterans benefits,
Vocational Rehabilitation, ASPIRE, Workforce Development, TRA, or other
employer or agency sponsorship). Failure to report such assistance may result
in a reduction or cancellation of FA awards, or repayment of aid already
received.

5. I have read the
Satisfactory Academic Progress (SAP) Policy and the
Refunds and Repayment Policy posted on the KVCC web site, I
understand that I must maintain SAP to continue receiving financial aid and that as per SAP regulations, I cannot receive financial aid for more than the
equivalent of three (3) years of full-time attendance. This period of eligibility is not extended when I change programs or withdraw and subsequently readmit to KVCC.

6. If I receive a Pell grant for full-time enrollment (12 or more credit hours per semester) for both the fall
and spring semesters, I will have received my maximum grant for the school year, and no funds will be left or a summer award.

|
|
7. Most grants and scholarships are credited to my account by the Cashier’s
Office in September (for Fall semester) and February (for Spring). I
understand that there is a 30-day delay from the start of term before grants
and federal Stafford loans are disbursed. Once my bill is satisfied, refunds
from the financial aid awards will be paid to me by KVCC check. The KVCC
Financial Aid Office does not process student loans or any other aid for the
summer term. If funds are available, some students MAY qualify for federal
work-study jobs (check with the Financial Aid Office). Pell grants are
available for students who have not already received their maximum annual Pell grant (see above).

8. I have received my Web ID and PIN numbers assigned by the Financial Aid Office
and understand that I am required to access my
financial aid
records on the internet.

9. I authorize the KVCC Financial Aid Office to discuss my financial circumstances and aid awards with other public
or recognized private agencies that also may be considering me for financial aid.

10. My signature below acknowledges that I have read and understand the information contained in this document.
Further, my signature affirms that all information submitted to the KVCC Financial Aid Office for the purpose of
determining my eligibility for financial aid is true and correct to the best of my knowledge.
